An information gathering model to identify health care professionals serving diabetics in North Carolina
Abstract (Summary)
Consumers seeking health care information often turn to sources that provide neither an accurate nor comprehensive selection of options available to them locally. An information gathering model was developed to assist NC Health Info with populating their database with individual providers for a single chronic disease, diabetes, at the county level. The model demonstrates that it is possible to develop a standardized approach to identify appropriate diabetes resources that can be applied routinely by the NC Health Info staff. The model can be adapted for use with other chronic diseases.
