Process of work of the management of the Strategy Healthof the Family: a study on management conception and its connection to health teams and the community.
This study mainly aims at analyzing the work process of city managers of the Family Health Strategy (FHS) and its connection to the health teams and the community in the area served by the Third Health Regional Coordination of Rio Grande do Sul (3ªCRS/RS). Transversal and explicative character study, presenting a predominantly qualitative nature. The data were collected through semi-structured recorded interviews, in the months of November and December 2006; along the managers of FHS belonging to city towns in the 3rd CRS/RS, with a total of 15 subjects. The data thematic analysis gathered a dialectic approach, following the steps of the pre-analysis, exploration and treatment of the material and results interpretation. The data was presented in three sub chapters, in which the first is connected to actions developed in the management work process of the FHS, synthesized in three sub themes: 1) Family Health Functioning/Organization Actions; 2) Permanent or continuous Education/Preparation Actions; 3) Evaluation and Monitoring Actions. From this studied work process, two management conceptions are learned, where in the first one relates to the classical/traditional management, in which we highlight the administration of material and human resources; the work standardization; and the imposition of responsibilities and work control. The second conception refers to the precepts of co-management, in which the sharing of decisions among managers, health teams and the users is gathered; the respect to the teams? autonomy; the negotiation of responsibilities and the evaluation and monitoring of the satisfaction from the teams and/or the community. The second sub chapter deals more specifically with the connection of FHS management with the health teams and thecommunity, being divided into two sub themes, which are: 1) Connection characteristics of FHS managers with the health teams and the community and 2) Connection purposes of theFHS managers with the health teams and the community. From this connection with the health teams and the community, it was possible to know who writes the contract, whose initiative it is and in which relation between time and organization the connection actions occur, in the sense of punctuality or continuation; and of programming or spontaneity. The third sub chapter describes the experience of giving back partial results of the research to the FHS managers, through the presentation of a Thematic Seminar. Concerning the results found, it was noticed that the work process of the studied FHS managers may assume a moredialogical character, in the sense of connection and negotiation with the subjects involved in the health production or a more vertical and controlled character. Just as we conclude it, it is seen that the work process of FHS managers in the 3rd CRS/RS presents strengths and weaknesses, related to the conception of management which structures its work. Therefore, it is suggested to reanalyze some practices, so that the co-management is the preferably used conception for the administration of Family Health, catching a glimpse of the production of usage values; i.e., to the satisfaction of human needs, either in the teams as well as in the community.
