Fungemia em pacientes portadores do vírus da imunodeficiência humana (hiv) e caracterização fisiológica dos fungos isolados

Abstract (Summary)
To investigate the occurrence of fungaemia, 530 blood samples were analysed between January 2004 and July 2004, from which 159 (30%) with HIV without AIDS and 371 (70%) with AIDS. All the patients were from Correia Picanço Hospital, Recife-PE, from which 468 patients were attended in ambulatory and 62 were intern. Fungemy was detected in four patients with HIV without AIDS and 20 with AIDS. In four HIV patients without AIDS fungemy was detected only after the culture of the samples. In AIDS patients fungemy was detected in seven samples after direct exam, in eight after culture and in five after direct exam and culture. From the blood of HIV patients without AIDS samples of Candida humicola, C. pelliculosa and Fusarium lateritium were isolated; from the blood of AIDS patients samples of C. curvata, C. humicula, C. parapsilosis, Cryptococcus albidus, C. neoformans and Histoplasma capsulatum were isolated. These findings present F. lateritium, C. curvata and C. humicula related with fungemy in this patients infected with HIV for the first time in literature. Each episodeof fungemy corresponded to only one fungus specie. In blood of HIV patients without AIDS levels of CD4 varied from 373 to 573 cells/mm3 and of CD8 from 807 to 1445 cells/mm3; in blood of AIDS patients the level of CD4 varied from 12 to 346 cells/mm3and CD8 from 261 to 1375 cells/mm3. Based on Logistic Regression model there is 152.57 more chances to occur fungemy in patients presenting skin lesions ans levels ofCD4 amp;#8804; 200 mm3 and there is no association between the isolated fungi and the levels of CD4 and CD8. Isolated fungi grew at 37°C. Protease activity was observed on the isolates using milk casein and gelatin as substrate. Phospholipase activity wasevidenced with soy lecitin as substrate, although this activity using egg yolk as substrate was not observed
