Acidente do trabalho : adesão a quimioprofilaxia dos trabalhadores da saude pos-exposição a material biologico humano

Abstract (Summary)
This study aims to verify the characteristics of workers, assess the characteristics of occupational accidents, and identify the adhesion to chemoprophylaxis against the human immunodeficiency virus, and follow-up tests of health workers after exposition to human organic fluids when there are risks of getting affected by infectious agents like mv, hepatitis B and C virus, and verifY the fellowship the adhesion to chemoprophylaxis against the human immunodeficiency virus and some variables. This is a descriptive epidemiological study, which was accomplished in the hea1th area of Sao Paulo, covering 21 districts. The population consisted of 172 health workers who were victims of accidents with some kind of virus in the year 2000, and 207 health workers in the year 2001. The facts were gathered through the notification records, and the Health State Secretariat of Sao Paulo established the investigation. The results showed that most part of the accidents occurred among women in 79% in the two years that were studied. The predominant age ranges ftom 20 to 40 in more than 90% of the hea1th workers who were victims of accidents with some kind of virus. The age rate was 34, in the 2 years that were studied. The nursery staff was the most affected professional class, 68% (in 2000), and 74% (in 2001). The period of the greatest occurrence of accidents was the day representing 81 % of the accidents. The most part of the biologica1 material that was involved in accidents occurred with blood in 87%. The percutaneous exposition represented 88% (in 2000), and 91% (in 2001), the agents that caused it were needles in 72% ( in 2000), and 77% (in 2001). The serologica1 status of the patients who were sources for mv represented more than 70%, However, the patients who were sources for hepatitis B and C were lower than 25%,more than 70% of the hea1th workers who were victims of accidents with some kind of virus referred complete scheme for vaccine against hepatitis B. In relation to adhesion to chemoprophilaxis against mv, in 57% and 43% (in 2000 and 2001) of the victims of accidents with some kind ofvirus who needed QP, 12% ofthem refused medication in the first year of study, and 1,3% in 2001, therefore they took the medication more than 22 days. The only variable that had statistics association with adhesion was a source patients who was unknown, therefore this patient had the value p= 0,0138 in 2000 and p= 0,0332 in the year 2001. We verified that the follow-up tests had a good adhesion in the moment ofthe accidents and they had upper proportions of accidents to 85% in lhe two years they were studied, except for AntiHBs. After sometime (tests realized in 45 and 90 days), the rate of realization is very low (ftom 57% to 67%), when the proportions reach the end of the period offollow-up (sixth months after accidents), theyare upper than 75%, and lower than 80% demonstrating low coverage. This study showed the need to establish new strategies to interfere in the reality of health workers, so that it can decrease the occurrence of accidents and improve the adhesion to chemoprophilaxis against HIV, and to follow-up tests when it's necessary
